It's not the first time somebody has gouged out one of her eyes as a punishment.

Ione Sala will sit and hold a hand over the eye socket so she doesn't bleed out too much, and endure, and try not to think about healing her own eye, even though she can probably do that at least partially.  The first time you do that, it sets the choice between channeling positive and negative energy permanently, and she suspects she will choose positive energy but she isn't sure and should think about sometime when she's not in pain.
